1. Certificate of Immunization – Blue We cannot admit your child without proof of immunization. The State of Montana requires the following immunizations for children entering preschool:
4 doses of DPT vaccine 3 doses of polio vaccine 1 dose of MMR vaccine 4 doses of Hib vaccine 1 dose of varicella vaccine
A child is exempt from an immunization only upon presentation of a written and signed statement from a physician stating that the immunization in question is medically contraindicated for that child.
2. Completed Health Assessment Form – Pink This form must be filled out by a physician, by a physician’s assistant, or by a nurse practitioner. It is required for admittance, but does not need to be redone every year.
3. Emergency Contact and Parental Consent Form – Yellow Consent for Emergency Medical Care and for Participation in Field Trips is required for acceptance into the program.
4. Personal Items These items, as well as the lunchbox and all outdoor clothing, should be clearly labeled with your child’s name.
A. A pair of slippers that can remain at school Fleece slippers with soft leather soles are best, but slip on tennis shoes and some discount store slippers work well, too. Please avoid Velcro fasteners; they stick to the carpet at circle time.
Slippers must stay on during movement activities, so be sure your child can skip and hop without losing them. Because children need to be able to put on and take off slippers independently, do not send tie shoes or buckle shoes to serve as slippers unless your child can tie or buckle.
Please do not send “crocs”, ballet slippers, oversized slippers, slippers with heads on the toes, or slippers that play music or make noise.
B. At least one complete change of clothing Every child needs these; three year olds need two changes.
C. A tote bag or a backpack All children are expected to carry their own things into and out of school. For little ones, tote bags are often easier to load and unload than backpacks.
